BayCare Health System is currently in search of our newest Social Service Specialist who is passionate about providing outstanding customer service to our community at Mease Dunedin Hospital. Established in 1937, Mease Dunedin Hospital is a 120-bed facility committed to improving the health of everyone it serves.
The Social Service Specialist is a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community.
- Provides therapeutic interventions through assessment,
- Coordination, referral and interdisciplinary planning
- Knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs.
- Possess a clear understanding of discharge and transition planning and linkage with community resources.
- Demonstrates skills in planning, organizing, and managing multiple functions
