BayCare Health Systems is currently seeking an experienced Case Manager to join our outstanding and compassionate team in Clearwater!
- Location: Clearwater
- Status: Full-Time, Exempt
- Shift Hours: M-F 8:00 - 5:00
- Shift: Days
- Weekend Work: No
- On Call : No
The Case Manager is responsible for:
- Provide direct behavioral case management services to adults or children with serious mental health conditions, substance use disorders, and/or those whose involvement in the child welfare system requires.
- Demonstrate competency in the domains of Engagement and Assessment; Service Planning and Delivery; Coordination, Linkage and Monitoring; Documentation; and Professional Responsibility.
- Coordinates the service delivery for individuals and provides community partners with up-to-date available referral resources to provide continuity of care.
- Effectively maintain and manage their assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
- Possess a clear understanding of the service delivery system in accordance with regulations and district policy.
- Provide services at the time and location that is most appropriate to the person served.
- Travel is required.
- May also be required to conduct urine drug screening and transport clients, depending on the program.
- Some programs may also require a 24 hour on-call rotation.
- Perform other duties as assigned.
Required Education:
- Bachelor’s degree related to related fields, Social Work, Counseling, Psychology Criminal Justice, Health Education, Special Education OR Other Human Services Related Field.
Experience:
- For targeted case managers working with children have 1-year full-time experience working with children with behavioral health concerns.
- For targeted case managers working with adults have 1-year full time experience working with adults with behavioral health concerns.
- Preferred 1 year of full-time experience with the identified population
- Targeted Case Managers required to bill Medicaid.
Required Certification/License:
- For targeted case managers (CBHCM) Certified Behavioral Health Case Manager certification must be obtained through the Florida Certification Board (FCB) within one year of employment.
