Role Description This full-time Hospice Social Worker role is a hybrid position based in Salisbury, NC, with a mix of on-site work at care settings and some work-from-home flexibility for documentation and telehealth support. The Hospice Social Worker will provide psychosocial assessments, care planning, and emotional support to patients and families facing serious illness and end-of-life decisions. Day-to-day responsibilities include coordinating services with the interdisciplinary hospice team, facilitating planning and transitions of care, and offering grief and bereavement counseling. The role involves advocacy for patient preferences, connecting families with community resources, and ensuring care is aligned with ethical standards and regulatory requirements. The Hospice Social Worker will maintain accurate clinical documentation, participate in team meetings, and contribute to continuous quality improvement in hospice and palliative services.
Qualifications
- Candidates should possess strong Medical Social Work and Social Work skills, including psychosocial assessment, care planning, and advocacy.
- Candidates should possess End-of-Life Care skills, including familiarity with hospice philosophy, advanced care planning, and palliative support.
- Candidates should possess Discharge Planning skills, including coordination of services, transitions between levels of care, and resource referral.
- Candidates should possess Grief Counseling skills, including bereavement support for residents, families, and caregivers.
- Relevant qualifications such as a master’s degree in social work (MSW) or related field, and appropriate state licensure or eligibility.
