BAYADA Home Health Care is seeking a compassionate, relationship-focused Transitional Care Manager to support the MGL division in Northern Colorado. This position may be filled by a qualified clinical or non-clinical professional who is organized, collaborative, and motivated to help clients and families transition safely and successfully to care at home.

The TCM partners closely with referral sources, patients, families, the marketing team, and BAYADA service-office teams. This role supports referral management, monitors hospital and health care portals, maintains account-specific ownership, and helps ensure timely communication from referral through the start of services.

At BAYADA, our mission is to help people have a safe home life with comfort, independence, and dignity. Our work is guided by The BAYADA Way: compassion, excellence, and reliability.

What you’ll do
  • Build and maintain professional, service-focused relationships with referral sources, hospital and facility contacts, physicians, discharge planners, case managers, patients, families, and other health care professionals.

  • Assist the marketing team with referral management and account-specific ownership across assigned referral sources.

  • Monitor hospital, health care, and referral portals for new and active referrals.

  • Manage incoming and current clients based on referral-source information and account-specific needs.

  • Make patient bedside and phone side contacts to support communication, education, and transition planning.

  • Serve as a knowledgeable resource regarding BAYADA home health services, referral processes, service expectations, and the transition from facility-based care to care at home.

  • Clearly explain the features and benefits of BAYADA services to referral partners, patients, and families.

  • Gather, verify, and communicate referral information accurately to the appropriate BAYADA service-office and clinical teams.

  • Coordinate the non-clinical aspects of the referral process and support timely communication among referral partners, patients, families, and BAYADA teams.

  • Collaborate with clinical, operational, and marketing partners to support appropriate and timely transitions of care.

  • Follow up with referral sources and internal partners to help ensure a smooth experience from referral through service initiation.

  • Maintain accurate referral information, follow-up activity, portal updates, and required documentation.

  • Participate in market-development and community activities that support ethical, service-focused growth.

  • Represent BAYADA professionally in hospitals, rehabilitation facilities, physician practices, community settings, and other referral environments throughout Northern Colorado.

  • Participate in team meetings, office planning, and other activities that support referral growth and excellent client service.

  • Perform other related duties as assigned or requested by the supervisor.

What you’ll need Required for all candidates
  • Strong alignment with The BAYADA Way: compassion, excellence, and reliability.

  • Ability to build trust and communicate professionally with patients, families, referral sources, and internal teams.

  • Independent problem-solving skills and sound judgment.

  • Ability to work across teams and collaborate productively in a fast-paced environment.

  • Strong attention to detail, organization, follow-through, and time-management skills.

  • Ability to self-manage, stay on task, and manage competing priorities.

  • Ability to learn and use hospital, health care, and referral portals.

  • Understanding of medical terminology or the ability to learn it quickly.

  • Proficiency with standard business technology, including email, internet-based systems, smartphones, and Microsoft Office applications.

  • Ability to travel throughout Northern Colorado approximately 25–50% of the time.

  • Ability to be logged in and available during business hours, including office hours when working outside the office.

Preferred qualifications
  • Bachelor’s degree or equivalent healthcare experience.

  • Home health or other health care experience.

  • Health care sales, marketing, community relations, referral coordination, or business-development experience.

  • Medical assistant or other patient-facing health care experience.

  • Experience working with hospitals, rehabilitation facilities, physician practices, discharge planners, or other referral partners.

  • Familiarity with hospital and health care portals.

  • Previous discharge-planning or care-transition experience.

  • Demonstrated success building and maintaining referral relationships.

A combination of education, experience, and relevant qualifications may be considered. Qualified candidates may be considered for either the clinical or non-clinical pathway based on their background and the needs of the MGL division.

Schedule and work expectations
  • Hybrid work model with approximately 25–50% field time throughout Northern Colorado.

  • Regular availability during business hours is expected.

  • When working outside the office, the employee must remain available during business hours, including office hours as needed.

  • Regular on-call coverage is not expected.

  • Training and ramp-up are expected to continue for approximately 60 days, with full competency targeted by the end of that period.
