What are the responsibilities and job description for the RN Home Health Case Manager - PRN position at BAYADA Home Health Care?
Overview:
Reporting to the Clinical Care Manager, the RN Case Manager is responsible for providing quality care to patients with Gulfside Healthcare Services, in the field. The field includes patients who reside in private residences, assisted living facilities, and skilled nursing homes. The RN Case Manager utilizes assessment, intervention, and education to maximize patient comfort and to enhance the quality of life for the patient/family. The RN Case Manager oversees and case manages the needs of the patients assigned to them. This includes providing supervision for the LPN's and CNA's, and other assigned staff in the field. The RN Case Manager is responsible for maintaining his/her own clinical and mandatory education requirements and for meeting the established standards of their profession as well as those accountable for his/her practice, assigned duties/patient care, and is an advocate for patients/families and their profession. EDUCATION AND QUALIFICATIONS:- Currently licensed in the State of Florida as a registered nurse
- Minimum of one year of nursing experience in skilled home health required
- Must have reliable transportation to perform tasks and responsibilities in a timely and appropriate fashion. Must be able to provide proof of automobile insurance and have a valid Florida driver's license
- Current BLS certification

Salary : $62,900 - $79,600
