Responsibilities
- Develop and implement individualized care plans that meet patient needs and goals
- Provide direct patient care, including medication administration, wound care, treatments, and procedures
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless transitions and optimal outcomes
- Utilize a state-of-the-art touch pad tablet to capture accurate and comprehensive information about patient care
Requirements
To be considered for this role, you must have:
- A current license as a Registered Nurse in North Carolina
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Prior home care experience strongly preferred
- Solid computer skills and prior experience with electronic medical records (EMR)
