We are looking for compassionate and dedicated Admissions RNs to join our team. This position will cover Monmouth, Middlesex, Somerset and Union Counties in NJ!

Experience with OASIS documentation and conducting Start of Care visits strongly preferred!

As an Admissions Nurse (RN) you will be prepared to succeed and to deliver exceptional care.

• Learn and expand your clinical skills caring for clinically complex level patients in the home
• Perform 1-3 Start of Care visits per day, focusing on 1:1 care with each patient
• Deliver 1:1 skilled nursing, intervention, and supportive care in accordance with individual care plan, including administration of medication, wound care, treatments, and comfort care
• Perform client assessments, provide education to the patient and/or their family, and monitor patients' condition
  
  
  

Qualifications:

• A current NJ nursing license in good standing.
• At least one year of related experience, preferably in Home Health.
• Comfortable utilizing an EMR; experience with HCHB a plus!
• Experience with OASIS and completing Start of Care visits.
  
  
  

Additional Information:

Base Pay, depending upon qualifications and subject to negotiation:

$60.00 - $65.00 per point
