The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver evidence-based interventions to improve mobility, function, and quality of life. Daily responsibilities include performing detailed assessments, setting measurable goals, providing manual therapy and therapeutic exercises, and progressing treatment plans based on patient response. The role involves close collaboration with referring providers, support staff, and other clinicians to coordinate care, ensure accurate documentation, and maintain regulatory compliance. The Physical Therapist will also educate patients and caregivers on home exercise programs, injury prevention, and long-term wellness, while actively participating in continuing education and mentoring opportunities.

Qualifications
- Strong clinical skills in Physical Therapy and Rehabilitation, including evaluation, treatment planning, and progression of therapeutic interventions.
- Demonstrated competency in Medicine and Patient Care, with a focus on safety, evidence-based practice, and interprofessional collaboration.
- Experience or interest in Home Care and continuity of care, including development and instruction of effective home exercise programs.
- Current state licensure (or eligibility) as a Physical Therapist in Massachusetts.
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited physical therapy program.
- Excellent interpersonal, communication, and documentation skills, with the ability to work effectively in a fast-paced clinical environment.
