Overall Summary:

The Medical Director – Specialty Programs provides senior physician leadership and executive level leadership for Bay Area Community Health’s (BACH) grant funded, pilot, and mission critical programs, ensuring alignment with HRSA Section 330 priorities, UDS reporting requirements, and federal, state, county, and foundation NOFO expectations. Serving as the clinical leader and accountable physician executive, this role oversees the full lifecycle of specialty programs—from concept development and grant writing through implementation, compliance, performance monitoring, and sustainability—while ensuring seamless integration into BACH’s clinical operations, quality improvement framework, data systems, and strategic plan. This position will play a critical role in integrating departments and service lines to advance whole-person care and further the BACH mission. The Medical Director – Specialty Programs will work closely with Medical Directors and Specialty Directors to strengthen cross-functional collaboration, align clinical priorities, and enhance coordinated care delivery across the organization. The position collaborates closely with executive and clinical leadership to advance innovative care models and strengthen organizational capacity, and is intentionally structured as part of BACH’s physician leadership pipeline with anticipated progression to Chief Medical Officer (CMO), contingent upon performance and organizational needs.

 

Essential Responsibilities:

Clinical and Program Leadership:

  • Serve as the clinical leader and physician executive lead for all special programs and grant-funded clinical initiatives across BACH.
  • Provide clinical oversight to ensure programs are evidence-based, patient-centered, culturally responsive, and aligned with BACH’s mission.
  • Lead integration and collaboration across departments to advance whole-person care and improve coordinated patient outcomes.
  • Act as a clinical resource for providers, care teams, and program staff supporting special populations.
  • Provide consultation on complex clinical cases related to special programs.
  • Maintain a limited direct clinical practice as assigned.
  • Oversee the Dental, Behavioral Health and Social Services, Chiropractic, Acupuncture and Vision departments.
  • Champion clinician success for mobile and school based clinics.
  • Oversee and ensure success of departmental performance metrics (e.g., access to care, provider productivity, quality outcomes, customer experience, budget performance and regulatory compliance), ensuring timely identification of gaps and implementation of corrective actions to support program sustainability.

Grant Development and Stewardship:

  • Partner closely with the Development Department to design, write, and support competitive grant applications.
  • Serve as the clinical lead and program manager for awarded grants.
  • Ensure full compliance with grant terms and conditions, including HRSA, state, county, and foundation requirements.
  • Lead or support preparation of clinical reports, progress updates, site visits, and audits.
  • Develop sustainability and scale plans for successful grant-funded programs.

Program Operations and Strategy:

  • Collaborate with Operations, Quality, Finance, IT, and Data teams to operationalize special programs.
  • Support development of policies, procedures, workflows, and protocols for special programs.
  • Identify opportunities for program expansion, innovation, and new funding aligned with community needs.
  • Represent BACH in external partnerships and community collaborations.

Quality, Compliance, and Performance:

  • Lead or support quality improvement initiatives tied to special programs.
  • Monitor key performance indicators related to grant objectives.
  • Participate in peer review, utilization review, and quality committees as assigned.
  • Ensure programs meet applicable clinical, regulatory, and accreditation standards.

Leadership Development:

  • Work closely with the Chief Medical Officer on clinical strategy, governance, and leadership development.
  • Support provider engagement, coaching, and mentoring related to special programs.
  • Participate in executive leadership meetings, budgeting, and organizational planning as assigned

 

Secondary Responsibilities:

  • Attends workshops, training, and meetings as needed, and as requested.
  • Performs other duties as assigned by supervisor.
Qualifications:

Qualifications:

Required Education, Experience, Licenses/Certifications:

  • Medical Doctor (MD) or Doctor of Osteopathy (DO).
  • Minimum 5-10 years of experience as a practicing physician.
  • Valid California Medical License in good standing.
  • Board Certified in Family Practice, Internal Medicine, or related specialty.
  • Minimum five years of experience in program leadership or grant-supported initiatives.
  • Working knowledge of HRSA, UDS, and safety-net healthcare compliance requirements.
  • Current DEA license.
  • Current Basic Life Support (BLS/CPR) certification.

Preferred Education, Experience, Training:

  • Master’s degree in Public Health (MPH), Health Administration, or related field.
  • Five years of leadership experience in an FQHC or community health setting.
  • Direct experience serving as a clinical or program lead on grant-funded initiatives.
  • Demonstrated success collaborating with Development teams.
  • Experience leading interdisciplinary teams.
  • Bilingual skills preferred.

 

Certifications/Licenses:

  • Board certified or board eligible in Family Practice, Internal Medicine or related specialty.
  • Valid California Medical License to practice medicine.
  • Current DEA and CPR certificates.
  • Leadership certification preferred.

 

Physical Demands:

This position requires:

  • Sitting, walking, and standing associated with a normal medical, clinical and administrative office environment, with some bending, reaching, stretching, and lifting up to 25 pounds.
  • Manual dexterity for tasks involving standard office equipment and computer keyboard, including repetitive movements, such as typing.
  • Traveling to work sites, including some exposure to outside elements.
  • Exposure to potentially dangerous materials and situations, requiring adherence to extensive safety precautions and possibly the use of protective equipment

Directors and Providers in areas of specialized practice and expertise
