Position Summary:
The Clinical Dietitian is responsible for assessing, planning, and implementing nutritional care programs to ensure optimal health outcomes for patients and residents. This role involves evaluating the nutritional needs of individuals, developing evidence-based dietary plans, and providing education and counseling to support medical and nutritional goals. The Clinical Dietitian collaborates with interdisciplinary teams to promote quality patient care in compliance with regulatory and clinical standards.
Key Responsibilities:
- Assess patients’ nutritional status based on medical history, laboratory data, anthropometric measurements, and clinical condition.
- Develop, implement, and evaluate individualized nutrition care plans according to physician orders and patient needs.
- Provide nutrition education and counseling to patients, residents, and families.
- Monitor tolerance and effectiveness of nutrition interventions and modify plans as needed.
- Collaborate with physicians, nurses, speech-language pathologists, and other care team members to ensure coordinated care.
- Participate in interdisciplinary rounds, care conferences, and quality improvement initiatives.
- Document all assessments, interventions, and outcomes accurately in the electronic medical record (EMR) system.
- Ensure compliance with state, federal, and facility regulations regarding nutrition and food service.
- Provide guidance to food and nutrition staff to support dietary service operations and menu development.
- Maintain up-to-date knowledge of current clinical nutrition guidelines and best practices.
Qualifications:
- Education: Bachelor of Science degree in Human Nutrition and Dietetics required.
- Certification:
- Registered Dietitian (RD) credentialed with the Commission on Dietetic Registration (CDR) or Registration-Eligible (required).
- Certified Dietitian/Nutritionist (CDN) in New York State preferred.
- Experience:
- Minimum 1 year of experience in an acute care or long-term care setting preferred.
- Technical Skills:
- Proficiency in PointClickCare, Meditech, Microsoft Word, Excel, and Outlook.
Job Type: Contract
Pay: $2,350.00 - $2,400.00 per week
License/Certification:
- BLS Certification (Required)
- Registered Dietitian (Required)
- NY Dietician Certification (Preferred)
