Responsibilities:
Under the oversight of the Operations Manager, the Community Health Navigation Coordinator organizes and implements administrative systems and procedures performing necessary support duties. The primary responsibility of this role is timely and accurate collection, entry and assignment of Medicaid Health Home referrals serving as the gateway of all member enrollment. Daily reviews of assignment files, data entry, and case assignment are among the several tasks an individual in the role will complete.

The Coordinator will also verify and update information by accessing multiple systems and assures that all members, partners and program stakeholders are given appropriate information, prompts and courteous service with an emphasis on maintaining member confidentiality. As part of this role the Coordinator conduct outreach to potential members to verify eligibility and assure engagement, while provided appropriate contact to referral sources on there referrals. In addition, the Coordinator will track all referrals, outreaches, assignments and initial outreaches, pull data and report information as needed. This role performs a wide variety of tasks associated with referral/member tracking to be compliant with program requirements.
  • Point of contact for referrals via email, fax, phone and Health Commerce System
  • Collaborate with partners for appropriate assignment based on potential members diagnoses, and eligibility needs.
  • Complete outreach for potential members referred from Managed Care Organizations, Emergency Departments, and other providers where eligibility will need to be determined.
  • Contribute to the team on projects and development of policy and procedures.
  • Provide administrative support to project leadership, work-groups, committees, stakeholders and members:
    • Schedules routine meetings and obtains meeting space
    • Perform various clerical or administrative functions
    • Record Meeting Minutes
    • Organize digital folders and maintain up-to-date records
  • Promptly obtains all missing and/or correct inaccurate information by establishing lines of community with patients, service providers and partner agencies
  • Attend program conference calls, take notes, and distribute call summaries
  • Assure timely submission of data to requesting entities as needed
  • Maintain tracking mechanisms to record programmatic questions, requests, and patient tracking
  • Accurately enters all patient information into Netsmart CareManager™ application
  • Use of multiple systems to verify patient demographics and information.
  • Adheres to HIPAA confidentiality regulations 100% of the time as observed by manager
  • Maintain documentation and statistics on activities to comply with reporting requirements
  • Maintain highly confidential material and performs high confidential secretarial duties for Project Staff as needed
  • Comply with all regulatory agency requirements
  • Demonstrates independent judgment and critical thinking skills, within scope of job responsibilities, in the absence of direct supervision.
  • Demonstrates efficient time management skills, demonstrated by task completion
  • May be required to perform the duties of other employees, including supervisors/managers, in their absence
  • May be required to perform duties and responsibilities not listed in this description, on a temporary or long-term basis
  • Performs similar or related duties as requested or directed
Qualifications:

Education:

  • High School Diploma or GED, preferred
Experience/Qualifications:
  • Minimum 5 years working in an office environment, required
  • Minimum 1 year experience collecting and disseminating information with attention to detail. Experience using Microsoft Word, Excel, Access,
    and Outlook, preferred
  • Minimum 1 year experience in the Medicaid Health Home, preferred

Licensure/Certificates:

Physical Requirements:

  • Sedentary Work - Exerting up to 10 lbs of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Sedentary work involves sitting most of the time. Jobs are sedentary if walking or standing are required only occasionally, and all other sedentary criteria are met.
