Position Summary
Basis Health is seeking a dedicated and experienced Nurse Practitioner (NP) to provide mobile primary care to a high-risk, elderly patient population and general population. This is a position offering flexibility, autonomy, and the opportunity to make an immediate impact on seniors and the general population who often struggle to access traditional healthcare.
The ideal candidate is compassionate, self-motivated, and comfortable practicing independently in a mobile setting.
Responsibilities
- Provide comprehensive primary care to elderly and medically complex patients in their homes or residential communities.
- Manage chronic conditions (diabetes, CHF, COPD, hypertension, dementia, chronic pain, etc.).
- Perform acute visits, health assessments, and follow-up evaluations.
- Develop individualized care plans focused on improving health outcomes and reducing avoidable hospitalizations.
- Coordinate patient care with Basis Health physicians, medical assistants, and support staff.
- Order and interpret labs, imaging, and diagnostic testing.
- Prescribe medications appropriately and responsibly.
- Document encounters efficiently using Basis Health’s mobile EHR platform.
- Provide patient and family education in a compassionate, easy-to-understand manner.
- Travel to patient homes within assigned service area.
Qualifications
- Active Nurse Practitioner license in the state of practice.
- Minimum 1 year of NP experience (primary care, geriatrics, or internal medicine preferred).
- Experience managing complex or high-risk patients strongly preferred.
- Active DEA license or eligibility.
- Ability to work independently in the field with remote physician support.
- Reliable transportation and willingness to travel locally to patient homes.
- Strong communication, critical thinking, and time-management skills.
- Compassionate and patient-centered approach to care.
