Job Summary
The Case Manager provides patients of Basecamp Recovery Center with comprehensive linkage to community resources to support recovery and overall wellbeing. This includes assistance with assessments, childcare and education, clothing, food, transportation, financial stability, and other needs. The Case Manager also serves as an advocate on behalf of patients to ensure access to appropriate services and supports.
Essential Duties and Responsibilities
- Uphold the mission and values of Basecamp Recovery Center; maintain professional relationships with patients, staff, and community partners.
- Comply with agency policies and procedures, including patient rights, safety protocols, code of ethics, and all applicable state and local laws.
- Deliver high-quality substance use, mental health, and supportive intervention services to patients.
- Provide prevention, consultation, and educational services as directed.
- Serve as a liaison between patients and external agencies to ensure access to appropriate programming and services; make referrals to mental health, legal, and medical resources as needed.
- Develop and implement case management plans in collaboration with the clinical team, including assessments, goal setting, service planning, monitoring progress, and supporting patient independence.
- Attend patient meetings, court appearances, school meetings, and other appointments as needed to support patient advocacy.
- Maintain accurate, timely, and confidential case records and documentation, including progress notes and collateral correspondence, in accordance with agency and regulatory standards.
- Support colleagues by assisting with caseloads during absences and collaborating on group case management when requested by the supervisor.
- Actively participate in supervision, case management meetings, treatment team meetings, interventions, and other agency-required meetings.
- Stay informed of emerging treatment strategies and best practices through professional development and continuing education.
- Contribute to Basecamp Recovery Center’s Quality Improvement activities, including peer reviews, compliance with OhioMHAS standards, and agency quality assurance guidelines.
- Perform other related duties as assigned.

Education and Experience
- Bachelor’s degree in Counseling, Social Work, or related field preferred
- Active licensure required: CDCA, LSW, LPC, LCDC-II, or LCDC-III
- Broad knowledge of community and social service systems
- 3–5 years of experience working with homeless or underserved populations preferred
