Summary of Position:
Provides professional nursing care to home health and hospice patients in a variety of settings including their own home, SNF/LTC, and Acute Care settings utilizing established policies and procedures, ensuring safe and effective care. The RN Case Manager assesses, develops, implements, plans, and evaluates care in collaboration with the interdisciplinary care team, in accordance with CMS, State regulations and The Joint Commission guidelines for Home Health and Hospice. Services are provided based on the age of the population served; Adults at least 18 years old through geriatric (65 years or older).
Qualifications
Education:
● Completion of an accredited Nursing program.
● Bachelor’s degree in Nursing (BSN) preferred.
Experience:
● One year of acute care experience in a hospital setting or similar healthcare setting preferred
● One year home health and hospice experience preferred
● In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred
Knowledge/Skills/Abilities:
● Proficient computer skills as are required to document, communicate and enter information into the electronic medical records system.
● Requires critical thinking skills, decisive judgment and ability to work with minimal supervision.
● Must be able to work in a stressful environment and take appropriate action.
● Excellent oral and written communication skills.
● Excellent interpersonal skills.
● Ability to collaborate with various departments in and outside of the Barton Health System in order to provide consistently exceptional patient care.
● Attention to detail, meticulous development and maintenance of regulatory paperwork.
Certifications/Licensures
● Current California and Nevada RN license or ability to obtain prior to start date
● Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date
● Valid California or Nevada Driver’s license (depending on state of employee’s residence) or ability to obtain prior to start date

Essential Functions
1. Provide consistently exceptional care at all times.
2. Demonstrates the knowledge and skill necessary to identify and meet the individualized needs of all patients served.
3. Directs the delivery of nursing care and verifies that appropriate measures are instituted for each patient for whom clinician is responsible. Initiates appropriate preventative and rehabilitative nursing measures.
4. Provides direct patient care, evaluates outcomes, consults with others as required and adjusts care plan as necessary to ensure optimal outcomes.
5. Performs initial assessment and repeat assessments at designated and appropriate intervals on all assigned patients, recognizing disturbed physiology and abnormal parameters.
6. The patients learning needs, abilities, preferences and readiness to learn is assessed. Effectively educates patients and families.
7. Assesses pain levels on all patients:
a. Upon initial assessment
b. Reassesses according to standards
c. Evaluates location, severity, intensity and quality of pain.
d. Uses the approved pain scale for measurement
e. Reevaluates pain level in response to intervention/ treatment
8. Conducts the comprehensive overview of Hospice services, including evaluating the appropriateness and patient family acceptance of terminal process. Provides patient and family teaching on Hospice philosophy.
9. Provides education to community and/or other healthcare professionals regarding Hospice.
10. Monitors and ensures accuracy of documentation in patient medical record. Assures patient records are complete and accurate.
11. Medication administration is safe, accurate and timely
12. Supervises the day-to-day work of home health aids.
13. Required to work on-call shifts and respond to Emergency and urgent patients needs 24 hours a day.
14. Responds to the needs of the department by performing other duties, as necessary.
