Clinical Pharmacist- Full Time- 4-10's

Summary of Position: 

The Clinical Pharmacist reviews, assesses and intervenes in patient medication therapy, processing medication orders, dispensing medications, providing drug information to health care professionals and education to patients. This position ensures that the pharmacy provides optimal clinical services on a daily basis, promoting desirable patient outcomes through safe and cost-effective drug therapies.

Qualifications

Education:

• Pharm.D. required

Experience:

• Two to four years clinical pharmacy experience in a hospital setting preferred

• Completion of PGY 1 residency or equivalent experience preferred

Knowledge/Skills/Abilities:

• Knowledge of hospital/clinical practice and service

• Computer literacy in common office applications

• Knowledge of electronic medical record (EMR) database or equivalent preferred; Epic experience preferred

Certifications/Licensure:

• California Registered Pharmacist license required.

• Nevada Registered Pharmacist license preferred.

• CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date.

• ACLS certification required or obtain within 90 days upon hire.

Essential Functions

1. Provides consistently exceptional care at all times.

2. Monitors and assesses drug therapy of hospitalized patients, performing interventions and recommendations as indicated.

3. Performs pharmacokinetic dosing for specified medications. Enters relevant medication and laboratory orders as necessary.

4. Participates as an active member of the multidisciplinary rounding team.

5. Attends emergency situations (full traumas / code blue) to provide pharmaceutical support and expertise.

6. Performs patient counseling on new therapies or discharge medications as indicated.

7. Responsible for the safe and accurate distribution of medications to patients being cared for at Barton Hospital, and for the distribution of medications to Barton Clinics. 

8. Supervises and oversees the daily activities of pharmacy technicians.

9. Responsible for overseeing compounding activities compliant with USP <797> and <800> standards. 

10. Prepares, mixes and compounds sterile medications adhering to aseptic procedures. Includes chemotherapy drugs, and other hazardous drugs, using closed system transfer devices and/or negative pressure aseptic techniques.

11. Facilitates the development and implementation of treatment guidelines, protocols, formulary changes. 

12. Performs monthly medication area inspections to ensure compliance with all relevant hospital policies, best practices and regulatory requirements.

13. Provides on-call pharmaceutical services after hours.

14. Responds to the needs of the department by performing other duties, as necessary.
