Overview

Job Summary

Provides direct, real-time clinical and operational pharmacy services within the Emergency Department and designated critical care areas to support safe, effective, and timely medication use. Works as an integrated member of the interdisciplinary healthcare team to deliver patient care in high-acuity and time-sensitive situations.

The role is primarily responsible for hands-on patient care activities, medication order processing, preparation, and dispensing, and is driven by patient volume, emergent events, and physician orders. Clinical judgment is exercised within established protocols, physician-directed care plans, and organizational policies.

Job Responsibilities

  • Reviews, verifies, processes, prepares, and dispenses medication orders in accordance with established policies, protocols, and provider orders.
  • Prepares sterile and non-sterile compounded medications following regulatory and safety standards.
  • Ensures appropriate medication selection, dosing, and monitoring based on patient-specific factors (e.g., age, weight, renal/hepatic function) within established clinical guidelines.
  • Provides drug information and medication recommendations to healthcare providers using approved references and protocols.
  • Documents clinical and operational activities as required.
  • Provides bedside pharmacy support in the Emergency Department and other designated critical care areas.
  • Responds in person to emergency situations, including cardiac arrests, trauma activations, stroke alerts, sepsis alerts, and other high-acuity events.
  • Participates in emergency response activities (e.g., ACLS), including:

Medication preparation and compounding

Dose calculation and verification

Real-time medication support during resuscitation

  • Prepares, verifies, and dispenses high-risk and time-sensitive medications (e.g., rapid sequence intubation agents, vasopressors, emergency infusions).
  • Provides medication-related recommendations within established emergency protocols, order sets, and physician-directed care plans.
  • Supports protocol-driven care such as sepsis, stroke, anticoagulation reversal, and other emergent treatment pathways.
  • Assists with medication reconciliation and transitions of care processes in the Emergency Department as applicable.
  • Work is shift-based and dependent on patient care demand, including unpredictable and emergent situations.
  • Maintains active involvement in tasks until completion or appropriate handoff, including during emergencies or high-acuity events.
  • May remain on duty beyond scheduled shift end when necessary to safely complete active patient care responsibilities or emergency responses.
  • Perform duties that are task-oriented, time-sensitive, and directly tied to patient care and operational workflows.

Minimum Required Experience

3 years of Clinical Experience

Minimum Required Education

Bachelor of Science (B.S.) or Doctor of Pharmacy (Pharm.D.).

Preferred/Desired Education

Post-graduate education in health-system pharmacy services

Minimum Required Training

PGY-1 Pharmacy Residency

Preferred/Desired Training

PGY-2 Pharmacy Residency and/or Pharmacy Board Certification, BLS and ACLS certifications and/or equivalent experience of greater than three years.

Minimum Required Special Skills

Computer literacy, interpersonal skills, general clinical awareness. Advanced communication skills and computer skills, including Microsoft Office applications. High-level cognitive ability to interpret and assess written medication orders to ensure optimal medication use. Teaching skills and qualified to precept residents, students, interns and externs.

Preferred/Desired Special Skills

Membership with state and/or national hospital pharmacy associations.

Minimum Required Licensure

Current, active, unrestricted pharmacy licensure for appropriate state or eligibility for licensure. Must acquire licensure within 120 days of hire date. PHARMACIST-FOR APPROPRIATE STATE
