**Responsibilities**
The primary focus of this position is to conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours. This includes triaging patient needs over the phone, responding to crisis calls, and making initial nursing evaluations.
You will also be responsible for performing after-hours visits, identifying patient and family needs, and documenting problems and responses to care.
