Job Summary
Baptist Health is looking for a Registered Nurse, Case Manager at Baptist Medical Jacksonville - Downtown campus. This is a PRN Day shift opportunity working Monday - Friday in the In-Patient Case Management Department at Baptist Jacksonville - Downtown campus.
As a Case Manager, you are responsible for:
- Care coordination for hospitalized patients.
- High risk screenings and assessments.
- Coordinating post-acute care services with proper documentation.
- Collaborating with multidisciplinary team members.
Experience Requirements:
- 1 year case management experience
Education & Credential Requirements:
- Associate's Degree Nursing
- Basic Life Support (BLS)
- Driver's Licensed (DMV)
- Active licensed Registered Nurse (RN)
