Location: Bangor Nursing & Rehabilitation Center | Bangor, ME

Are you looking for a role where your expertise meets a "sacred trust"? At Bangor Nursing & Rehab Center, we believe providing patient-focused care is more than just a job—it’s a genuine calling. As a member of the Covenant Health family, we are committed to being a reassuring, knowledgeable presence for our residents and their families.

We are looking for a compassionate, high-energy Director of Social Services to lead our department and ensure our residents' emotional and social needs are not just met, but championed.

Why Join Our Team?

  • Mission-Driven: Work for an organization that values integrity, excellence, and a heart for service.
  • Leadership Role: You won’t just be a staff member; you’ll be a key part of the management team, reporting directly to the Administrator.
  • Community Impact: Serve as a vital liaison between residents, families, and community agencies.

What You’ll Do:

As the Social Services Director, you will oversee the entire operation of the Social Services Department. Your day-to-day will include:

  • Advocacy: Acting as a mediator and advocate for resident rights and preferences.
  • Clinical Leadership: Completing social histories and psychosocial assessments (OBRA guidelines) and participating in interdisciplinary care planning.
  • Support & Education: Providing crisis intervention, emotional support, and education to families regarding Advance Directives, MaineCare, and discharge planning.
  • Quality Improvement: Developing departmental goals and ensuring we stay ahead of federal and state regulations.
  • Team Collaboration: Working harmoniously with nursing, medical, and administrative staff to create a holistic care environment.

What We’re Looking For:

  • Licensure: You must possess an active and valid Social Worker license.
  • Experience: A minimum of two (2) years of experience in a healthcare setting (Long-Term Care experience is highly preferred!).
  • The "Heart" Factor: We need someone with patience, a cheerful disposition, and the enthusiasm to handle difficult situations with tact.
  • Leadership: The ability to make independent decisions and a willingness to seek out new, innovative methods of care.

Work Environment & Physicals:

  • This role involves regular interaction with residents and staff throughout our well-lit, ventilated facility.
  • Physical requirements include sitting/typing for periods, lifting/moving up to 15 lbs., and navigating the facility to make daily rounds.
  • A flexible mindset is key—while we strive for a steady schedule, occasional weekends or holidays may be required to meet resident needs.
