Position Summary
The Per Diem RN delivers direct nursing care to hospice patients and their families, including RN visits, admissions, and recertification evaluations. This role ensures compliance with all state regulations and federal Conditions of Participation while supporting patients with dignity, comfort, and respect.
Key Responsibilities
- Provide direct nursing care through routine visits, admissions, and recertifications
- Perform comprehensive patient and family assessments and develop individualized care plans
- Conduct pain and symptom management assessments at every visit
- Communicate changes in patient condition to the RN Case Manager and physician as needed
- Maintain accurate, timely documentation in accordance with hospice policies
- Participate in interdisciplinary team (IDT) meetings
- Support patients and families during end-of-life care, including presence at time of death when possible
- Supervise LPNs and Home Health Aides as needed
- Ensure compliance with all regulatory, HIPAA, and infection control standards
- Participate in ongoing training, quality improvement, and professional development
- Current Registered Nurse (RN) license in good standing
- Minimum 2 years of nursing experience
- Strong clinical, communication, and interpersonal skills
- Ability to work independently with minimal supervision
- Basic computer proficiency (MS Word, Excel, EMR systems)
- Must pass background check and pre-employment requirements
- Previous hospice experience
- Prior supervisory or leadership experience
- Ability to lift up to 50 lbs and perform physical tasks (walking, standing, bending, etc.)
- Ability to travel to patient homes on a daily basis
- Comfortable working in varied home environments, including exposure to illness and other conditions
