Longwood Hospice is currently seeking a Registered Nurse (RN) Case Manager!
The RN Case Manager is responsible for providing nurse and hospice case management to hospice Patients and families. Implements nursing interventions and coordinates the Patient/family interdisciplinary plan of care. The RN Case Manager adheres to the regulatory process required by the State Board of Nursing and Federal Conditions of Participation.

Essential Job Functions and Duties:
- In conjunction with the Interdisciplinary Team (IDT), manage and coordinate the patient and
- Facilitate communication about changes in the patient’s status among IDT members and the patient’s
- Plan, coordinate, and deliver direct nursing care to patients and families according to the patient’s need.
- Visit each patient at least every 14 days in accordance with regulatory requirements.
- Maintain a minimum Productivity Standard according to Hospice Policy. Productive time consists of
includes coordination of care activities with families, physicians, and host nursing facility partners, such
as: TQMs, at-risk meetings, information presentations, or meet and greets.
- Perform a comprehensive assessment of the Patient/family and develop a Plan of Care and ensures the
- Assess patient needs for necessary therapies (PT, OT, and ST) and consult to assist in quality of life and
- Ensure that all care is provided consistently with respect for patient rights. Report any/all allegations of
- Communicate all aspects of patient care with SNF/caregivers.
- Supervise and document Home Health Aide Care Plans and document supervisory visits.
- Participate in regularly scheduled Hospice IDT meetings to coordinate the care of the patient and family,
- Participate in the on-call nursing rotation schedule.
- Complete other duties as assigned.
- Current license as a Registered Nurse (RN) in good standing with the MA State Board.
- Must have demonstrable competency with computers (MS Word, MS Excel, etc.)
- Solid verbal, written, and interpersonal communication skills are required.
- Experience working independently with minimal supervision is required.
- Candidates must successfully complete criminal background check and physical prior to completing
Preferred skills and/or experience:
- Previous experience in hospice preferred.
- Previous management or supervisory experience preferred.
- Must be able to lift up to 50 pounds. Frequent lifting and/or carrying of objects weighing up to 25
- Must be able to travel to facility sites daily.
Salary : $40 - $53
