Bostonian Nursing and Rehabilitation is seeking a Director of Social Services with a $5,000 Sign-On Bonus, to join and support our fantastic team caring for our short-term patients and long-term residents. Requirements:
Must possess a minimum of a B.A in Social Work, LSW or equivalent degree AND a current social work license.
Master's in Social Work (MSW), LCSW, or LICSW highly preferred.
1 years skilled nursing facility experience preferred
Job Duties:
Compiles social history data.
Participates in assessments, development and evolution of resident's care plan.
Responsible for planning and leadership interdisciplinary care planning weekly meetings.
Documents timely progress notes according to state and federal regulations.
Coordinates the discharge planning for residents.
Able to interpret and implement established social service policies, procedures and regulations.
Coordinates services of community agencies for continued delivery of care upon discharge.
