What are the responsibilities and job description for the Physical Therapist - Per Diem position at Bancroft?
- Provide direct and consultative physical therapy services, including evaluations, treatment planning, and interventions, in compliance with APTA standards, state regulations, HIPAA, and organizational policies.
- Conduct physical therapy assessments to determine individual needs, functional abilities, limitations, and rehabilitation goals within residential and program settings.
- Develop, implement, and modify individualized physical therapy treatment plans based on assessment findings and ongoing progress.
- Establish measurable, functional, and achievable therapy goals that support independence and daily living skills.
- Deliver individual, group, and consultative therapy services while promoting independence and functional mobility.
- Recommend, demonstrate, and train individuals and staff on the proper use of adaptive equipment, positioning techniques, and therapeutic interventions.
- Collaborate with interdisciplinary team members, families, guardians, and program staff to support coordinated care and ensure continuity of services.
- Provide guidance on environmental adaptations, functional strategies, and adaptive equipment to enhance participation and progress.
- Maintain accurate, timely, and professional documentation, including evaluations, progress notes, and treatment summaries, in accordance with IPP and regulatory requirements.
- Participate actively in team meetings and the Trans-Disciplinary Team (TDT) process, communicating assessment results and treatment outcomes to all relevant stakeholders.
- Provide training and education to staff and individuals served regarding physical therapy goals, techniques, and treatment plan implementation.
Pay: $58.00 per hour
