Job Purpose
The MDT (Multi-Disciplinary Team) Facilitator assesses, coordinates and manages the daily activities and information sharing of the Multidisciplinary Team. The facilitator’s main purpose is to support the delivery of services by the MDT and ensure its effectiveness and cohesion. This role involves strong organizational skills to support case coordination, information sharing, and outcome monitoring for child victims and their families.
Essential Job Duties
Graduation from an accredited college or university with a bachelor’s degree, preferably in Social Work or a related field.
Plus
A minimum of two years of relevant experience.
Preferred Education
A master’s degree is preferred but not required.
Knowledge, Skills, And Abilities
Possession of a valid license equivalent to a non-commercial, class C Maryland driver's license may be required for some positions at the time of appointment.
Proof of Licenses, Certifications and Education
Applicants are required to submit proof of licenses, certifications, and education beyond high school to meet the required qualifications of the position. Diplomas or transcript must show the applicant's major field of study. Copies and unofficial transcripts are acceptable.

EXAMINATION PROCEDURE
Applicants will be qualified based on an evaluation of the training and experience as stated on their application, which includes answers to the supplemental questions. Job Purpose

Essential Job Duties
- Effectively coordinate the MDT response to child abuse in their community.
- Build and maintain relationships among MDT members.
- Facilitating shared decision-making to ensure a coordinated, comprehensive response.
- Ensuring adherence to MDT agreements and protocols as directed by NCA accreditation.
- Providing a vital, unique skill set for understanding stages of team development, group dynamics, and key concepts for team resilience.
- Ensuring continuous care coordination across all disciplines that fosters a CAC best practice response
- Identifying gaps in service delivery across disciplines
- Implementing solutions to multidisciplinary challenges or gaps in services
- Recognizing sources of bias and identifying strategies for mitigating it
- Identify any emerging issues or notable trends impacting crime victims in our jurisdiction
- Applying strategies to build trust and psychological safety among MDT members
- Act as a central point of contact, manage confidential information, and ensure care plans are implemented effectively
- Tracking client pathways or case outcomes to meet NCA reporting requirements
- Assisting CAC Co-Directors with formally onboarding new team members
- Practice building powerful questions for increased dialogue and conflict resolution
- Schedule Diversion Signings by calling families and explaining the process of diversion agreements, setting up meetings and coordinating/facilitating the meeting with all parties.
