Role Description
This is a full-time on-site Social Worker role based in Baltimore, MD. The Social Worker will be responsible for assessing clients' needs, creating individualized service plans, and connecting individuals and families to available resources and services. Responsibilities also include supporting clients in navigating health and wellness programs, coordinating care with various agencies and healthcare providers, advocating for clients, and maintaining accurate documentation. The successful candidate will work closely with clients and partner organizations to promote equitable access to services, with a focus on improving overall health and well-being in underserved communities.
Qualifications
- Strong assessment, case management, and advocacy skills
- Proficiency in communication, counseling, and interpersonal skills
- Ability to collaborate with healthcare providers, community organizations, and other stakeholders
- Experience in maintaining detailed and accurate client records
- Knowledge of social work best practices, community resources, and health promotion strategies
- Bachelor's or Master’s degree in Social Work or a closely related field required
- Current licensure or eligibility for licensure as a Social Worker in Maryland
- Ability to work effectively in diverse communities and adapt to varying challenges
- Prior experience in public health or working with marginalized populations is a plus
