Responsibilities:
· Plan, develop, organize, implement and direct the Social Service programs of the nursing home.
· Supervise and direct the employees of the Social Service Department.
· Assure that Care Plans and documentation are updated and accurate.
· Act as the liaison between the Interdepartmental Team and the Social Workers to communicate important information between them

Requirement:
