Position Overview
We are seeking experienced Registered Nurses (RN) or Licensed Practical Nurses (LPN) to join our team as per diem independent contractor Care Managers. This role is ideal for a clinically skilled nurse who is passionate about working with older adults in home and community settings. Hours are flexible and based on client need and contractor availability.
Responsibilities
- Coordinate care across medical providers, home care agencies, community resources, and senior living facilities
- Monitor client status and update care plans as needs evolve
- Communicate with families and serve as a trusted liaison between clients and their care team
- Accompany clients to medical appointments as needed
- Assist with transitions of care (hospital to home, home to facility, etc.)
- Document all care coordination activities accurately and in a timely manner
- Maintain client confidentiality in accordance with HIPAA regulations
Qualifications
- Current Massachusetts RN or LPN license in good standing (required)
- Minimum 2 years of clinical nursing experience
- Strong assessment and critical thinking skills
- Excellent written and verbal communication
- Comfortable working independently in clients' homes and community settings
- Valid driver's license and reliable transportation
- Experience with older adults strongly preferred
- Certification in Geriatric Care Management (CGCM) or as a Dementia Practitioner (CDP) a plus, but not required
Compensation & Structure
- Independent contractor (1099) engagement
- Per diem / as-needed schedule — flexible hours based on client caseload
- Competitive hourly rate commensurate with experience
- Work closely with a supportive, experienced clinical leadership team
Pay: $50.00 per hour
