Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and document patient vital signs and health status using Electronic Health Record (EHR) systems
- Support patients with medication administration and catheter care following medical guidelines
- Provide companionship and emotional support to individuals with dementia, Alzheimer's disease, or other cognitive impairments
- Assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to maintain a safe and hygienic environment
- Use medical terminology accurately when documenting patient records and communicating with healthcare teams
- Operate medical equipment like Hoyer lifts safely and effectively during patient transfers
- Follow HIPAA regulations strictly to protect patient privacy and confidentiality
- Collaborate with nursing staff on hospice, memory care, or long-term care plans as needed
Requirements
Bilingual : English & Spanish
- Valid certification as a Home Health Aide or equivalent credential; certification must be current and active
- Prior experience in assisted living facilities, nursing homes, or home health environments preferred
- Knowledge of medical terminology, vital signs monitoring, medication administration, and infection control protocols
- Experience working with individuals with developmental disabilities or dementia is highly desirable
- Ability to operate EMR/EHR systems for accurate documentation of patient care activities
- Strong communication skills; bilingual abilities (English/Spanish) are preferred to serve diverse client populations
