Speech Language Pathologist (Home Health) (Part time/PRN)

Part-time/PRN licensed Speech Language Pathologist (SLP) position assessing, diagnosing, treating, and preventing communication and swallowing disorders in patients' homes.

Work with patients experiencing cognitive-communication and swallowing issues due to stroke, injury, illness, surgery, or dementia.

• Conduct initial assessments and develop individualized treatment plans.
• Collaborate with the care team to ensure seamless continuity of care.
• Document patient progress and treatment details in the health information system.
  
  

Requirements include current SLP license and valid driver's license. The role involves travel and requires relocation to State College, PA before starting.
