Palliative Care Nurse Practitioner

We are seeking a full-time Palliative Care Nurse Practitioner (NP) to join our team in Milwaukee, WI, and surrounding areas.

• Responsible for developing and managing a caseload of palliative patients, providing comprehensive care including diagnosis, treatment, medication management, and assessments.
• Travel to patients' homes or facilities to deliver personalized, compassionate care, ensuring comfort, dignity, and peace at the end of life.
• Maintain expertise in hospice and palliative care practices while collaborating with a multidisciplinary team.
• Requirements include an active APRN and RN license, ability to obtain an NPI number, and willingness to collaborate with the Medical Director. 1-2 years of nursing experience and a compassionate, integrity-driven approach are preferred.
