Nurse Practitioner/Physician Assistant

We are seeking a Nurse Practitioner or Physician Assistant to join an experienced team serving the geriatric population in Skilled Nursing, Rehab, Assisted Living, and retirement communities. This full-time position, working four days a week with approximately 16 patient visits daily, involves managing health issues, performing age-appropriate assessments, interpreting tests, and prescribing treatments in accordance with regulations and standards of care.

The provider functions independently with team support, assists with surgical care as needed, monitors intervention outcomes, and facilitates patient transitions between healthcare settings. Collaboration with multidisciplinary team members and providing educational resources to staff, patients, and families are key responsibilities.

Compensation ranges from $125,000 to $140,000 annually, with benefits including 401(k), matching, health/dental/vision insurance, HSA, paid time off, life insurance, and flexible scheduling. The role is in person and offers a rewarding opportunity to impact the geriatric community.
