Nurse Practitioner - Full time

The Nurse Practitioner is a key member of the clinical team, providing assessment, diagnosis, treatment planning, crisis intervention, medication management, staff consultation, and participation in group visits.

• Assists in care coordination and conducts comprehensive patient assessments.
• Develops and implements personalized care plans using decision support tools.
• Performs procedures appropriate to training level and confers with other providers regarding patient care.
• Supports epidemiological efforts and communicable disease control.
• Utilizes electronic health records effectively and collaborates with the care team.
• Participates in peer review, team training, and population-based care activities.
