Licensed Clinical Social Worker (LCSW)

We are hiring a Licensed Clinical Social Worker (LCSW) with geriatric behavioral health expertise for a growing provider supporting long-term care facilities across the Midwest.

• Position available in Union Grove, WI, offering part-time or full-time hours, daytime schedule, Monday to Friday, with flexible hours.
• Responsibilities include providing direct psycho-social services, conducting evaluations, diagnoses, and ongoing treatment through individual, group, and family therapies for nursing home residents.
• Collaborate within an interdisciplinary team to ensure quality care, document according to Medicare guidelines, and manage 10-15 patients daily.
• Qualifications include an active LCSW license, 1-2 years of experience, strong communication skills, and proficiency with technology.
• Preferred candidates have experience in healthcare settings like nursing homes and familiarity with Medicare documentation.
