Key Responsibilities
- Perform initial and ongoing assessments of mobility, coordination, strength, and functional status.
- Administer, evaluate, and interpret tests to set functional goals and outcomes.
- Develop and document plans of care outlining occupational therapy needs, goals, progress, and expected results.
- Deliver therapy services as prescribed, maintaining timely documentation and modifying treatment plans as needed.
- Manage discharge planning, including education on home exercise programs for patients and caregivers.
- Coordinate with healthcare team members, physicians, and families, serving as a resource for clinical staff.
- Complete OASIS assessments where permitted by regulation and document effectively to prevent claim denials.
- Supervise other personnel, such as OTAs and HHAs, as applicable.
- Degree in Occupational Therapy from an accredited program.
- Current, unrestricted state license as an Occupational Therapist.
- Valid CPR certification and driver's license.
- Preferred skills include strong organization, communication, attention to detail, time management, and experience in home health therapy and OASIS documentation.
