The Role
Lead our Payson NP-led clinic and partner closely with local PCPs, the community hospital, and senior-living communities to deliver timely, guideline-based cardiovascular care.
Schedule & Setting
- Hours: Monday–Friday, 8:00 a.m.–5:00 p.m.
- Setting: Outpatient clinic with limited same-day access blocks to reduce wait times; collaboration with AZ Cardiology hubs for EP/structural/cath/imaging.
What You’ll Do
- Evaluate and manage new and established patients with CAD, heart failure, arrhythmias (AF), hypertension, dyslipidemia, and cardiometabolic risk.
- Order, perform, and interpret clinic-based diagnostics (ECG, echo, stress, ambulatory rhythm monitoring, ABI); collaborate with cardiologists for advanced interpretation as needed.
- Titrate guideline-directed medical therapy (HF, HTN, lipid, AF anticoagulation) using established protocols.
- Coordinate post-discharge follow-ups, device checks, and transitions of care; close the loop with referring PCPs.
- Use telehealth when appropriate; contribute to quality initiatives (access, readmissions, guideline adherence).
- Provide patient education and shared-decision counseling centered on prevention and lifestyle.
Qualifications
- Arizona-licensed Nurse Practitioner (FNP, AGACNP, or AGPCNP) with DEA; prescriptive authority (or eligibility).
- Cardiology experience preferred (clinic, inpatient, or advanced primary care). Strong new grads with focused cardiology rotations considered.
- Current BLS/ACLS.
- Comfortable in an NP-led, protocol-driven environment; collaborative with cardiologists and primary care.
