Wellness Coach - Full Time – Cortez Integrated Health – Cortez, CO

Position Summary: The Wellness Coach supports patient wellness and health integration in outpatient clinics.  Wellness coaches are an integral part of a patient’s Care Team and work within multidisciplinary teams and other support staff.  This position is responsible for engagement of clinic patients into a variety of healthcare activities through population-based assessments and screenings and tracking patients’ follow-up and progress.  Wellness coaches focus on prevention, health education, brief interventions and referral to additional care when indicated.

Qualifications: Our ideal candidate for our Wellness Coach position will have their high school degree (or equivalent).  A bachelor’s degree in social work, Psychology, Counseling, or equivalent field is preferred.  Two years of volunteer or paid experience in a healthcare or social service setting is also preferred.

Salary: Starting pay is $17.50 - $19.60 per hour which considers preferred experience and preferred education described above.

Physical demands: Employee must occasionally lift and/or move 15 pounds up to 50 pounds on an occasional basis. Employee is frequently required to walk, sit, stand, or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employee must be able to travel between Axis locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate. Required Skills:

• Knowledge and ability to operate standard office equipment including scanner, computer, calculator, copy machine, fax, and multi-line telephone.
• Working knowledge of Windows-based computer applications including, MS Edge, Excel, Outlook, Teams, Word, Office 365, and ability to access the internet.
• Ability to be helpful and sensitive to the needs of all employees, while concurrently respecting corporate policies, procedures, and priorities.
• Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers.
• Ability to communicate job-related information to individuals and small groups.
• Ability to work independently with minimal supervision; plan own work schedule effectively, multi-task and prioritize in a fast-paced setting.
• Ability to multi-task, organize and prioritize workload in a sometimes-hectic environment with frequent interruptions.
• Ability to concentrate in sometimes hectic and stressful settings.
• Ability to read and understand written materials.
• Must regard work as confidential.
• Must have current driver’s license, qualify for commercial vehicle insurance and maintain a driving record that allows insurability with Axis’ insurance agency.
• Travel between clinics as required.
• Ability to take on additional duties when necessary.

Additional eligibility requirements:  Annual Flu immunization, Annual TB screening, de-escalation training within 90 days of hire, BLS certification required within 90 days of hire.
