Job Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate healthcare team. The ideal candidate will possess strong clinical skills, a passion for patient care, and experience working in diverse healthcare settings. As an LPN, you will be responsible for delivering high-quality nursing care, supporting patient assessments, and ensuring compliance with healthcare standards within our long-term care and rehabilitation environment.
Responsibilities
- As a LPN you will provide direct patient care by monitoring vital signs, administering medications, and performing routine assessments.
- Assist with medical procedures such as IV infusions, catheterizations, and sterile techniques while adhering to aseptic protocols.
- Document patient information accurately using Electronic Health Record (EHR) systems such as Point Click Care.
- As a LPN you will need to confidently collaborate with interdisciplinary teams to develop and implement individualized care plans focused on patient needs.
- Support patients with activities of daily living, including mobility assistance and nutrition management.
- Conduct triage assessments to prioritize patient needs in urgent or emergent situations.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient records and medical information.
Candidates should have prior experience working in various healthcare environments such as long-term care facilities, hospitals, or assisted living communities. Experience with critical care settings like ICU or Level I Trauma Centers is highly desirable. Familiarity with medical coding systems including ICD-9 and ICD-10, as well as proficiency in EMR/EHR systems such as Point Click Care, is preferred. Additional valuable skills include experience in hospice care, dementia or memory care, pain management, infection control practices, and working with individuals with developmental disabilities or behavioral health needs. A valid LPN license is required along with current CPR/BLS certification.
