Awixa Home of LI, Inc. is a not-for-profit 501(c) (3) New York State QRTP designated organization which licensed and monitored by New York State Office of Children and Family Services (NYS OCFS). Awixa was founded in 2001 to provide children & youth with a safe, nurturing and home-like environment where they will be empowered to change the world with their own two hands.

The primary goal at Awixa is to help our youth reunite with their families, or when there are no families to reunite, to empower our youth to be fully self-sufficient.

Awixa operates two separate nine bed group homes; one for adolescent boys ages 12 years to 21 years old, and one for adolescent girls ages 12 years to 21 years old.

The Group Home Case Manager will serve as a member of our treatment team and will report directly to the Clinical Supervisor. We are looking for a concerned, motivated, and capable individual dedicated to continuing our commitment to providing a supportive, structured environment for our residents. The Caseworker will schedule, track, and document all services provided for our youth, i.e. family visitation, medical, dental, optical, and therapeutic appointments. Proficiency in NYS Documentation system, Connections, a plus.

Qualification Requirements:

Must be dedicated individual interested in quality service provision for youth in care. Must have some understanding of the Family First Services Prevention Act as it relates to Congregate Care agencies.

Education : Bachelors Degree required in Psychology, Social Work, Mental Health, or related field

Previous Casework experience required

Experience with all aspects of NYS Connections database, Accumed, documentation, treatment plans, progress notes, and FASP writing.

Valid NYS Drivers license with a clean record required

Per Diem, Part-time & Full-time positions available
