What are the responsibilities and job description for the Mobility Mentor position at AVIVO?

The Mobility Mentor provides case management, career counseling, and follow along support to Avivo Institute students and job seekers at various training sites in the community. This role also provides intensive life coaching services, utilizing a research-based model to assist participants with setting personal goals related to economic self-sufficiency. The Mobility Mentor supports clients in the areas of economic success, power and autonomy, and increased understanding of their value within the communities where they live and work.
PRIMARY RESPONSIBILITIES
- Identify and market to individuals eligible for Avivo Institute training programs within Avivo and in our larger community; develop partnerships with referring agencies for program recruitment.
- Assist participants with goal setting, problem solving, direct support, intervention, and follow-through with personal goal attainment.
- Provide intensive case management and advocacy services including 1:1 support to connect participants to comprehensive support services as identified by each participant. May include assistance with transportation, housing, or medical needs; counseling; financial literacy; life skills; work clothing; work tools, and access to legal services. Make referrals to peer support groups, mentorship programs, and job or life coaching.
- Coordinate services with Avivo staff, project partners (as applicable), and classroom instructors.
- Communicate with relevant internal and external partners to review and assess participant progress and address any concerns that arise during the training and in job search.
- Create job leads and connections with employer partners with the goal of assisting participants to obtain and maintain competitive employment.
- Assist participants in preparing for interviews, completing applications, and creating or revising their resume.
- Coordinate participation in job clubs, workshops, or assistance with post-secondary education.
- Provide a minimum of 1 year of follow-up support to all participants.
- Maintain participant files including data entry in the required database(s); submitting reporting forms, and other data management as required in a timely manner.
- Attend and occasionally present at internal and external meetings such as: job fairs, community partner meetings, business partnership council, team meetings, or internal department meetings as needed to promote the program.
- May attend additional trainings and/or receive addition education as required to implement the full scope of services for the program.
- Attend all scheduled staff meetings and perform other duties as assigned.
- Actively participate in AVIVO’s Undoing Racism Plan
- Promote and support involvement in the organization’s commitment to undoing racism and reducing disparities
- Develop and complete individual annual diversity goal
- Ensure services provided are culturally responsive utilizing best practices

High School Diploma or GED

Minimum of 4 years in human services or related field experience.

If bachelor’s degree in human services, Social Services, or other related field held, only 1 year experience required.

Must have a valid, current driver’s license, proof of valid insurance and a clean driving record.
Desired:
