Our Aviva Blended Case Manager will be a dedicated and compassionate individual to help our client's obtain access to the support services families need. The role of our BCM is to be a stable resource, available to assist the family as challenges arise both day to day and long term. The BCM will work to connect the family and or individual with all resources that they may need.

Job Duties:

  • Meet with individuals and family members in their homes as well as in the community
  • Establish and maintain effective working rapport with individuals, associates and agency personnel
  • Link individuals and families to needed services, resources and programs to positively impact an individual's overall well-being
  • Collaborate with other members of the Aviva treatment team, as well as other providers and organizations
  • Complete recovery-oriented and thorough documentation within the electronic heath record and client chart
  • Adhere to BCM departmental productivity standards
  • Provide community-based Blended Case Management services focused primarily in Delaware County

Job Requirements:

  • Bachelor's degree, which includes or is supplemented by at least 12 college credits in Human Services, Psychology, Social Work, Psychology, Sociology or related field
  • At least 1 year direct client care experience in a behavioral health provider setting preferred. Internship and volunteer experience will be considered
  • Computer proficient
  • Reliable car
  • Ability to multi-task and prioritize by staying organized
  • Ability to perform the physical requirement of the job description
  • Ability to handle a case load of at least 15 clients
