Care Management Director (LPC, LCSW or LMFT)

Avita Community Partners is currently looking to fill the role of Care Management Director. This exciting opportunity will be perfect for the person who loves quality management, utilization management and review and continuous quality improvement (CQI). This position is robust and touches many important agency functions. Time management, attention to detail and keen organization is a must for the perfect candidate.

This key administrative position will oversee the work of the Care Management Department. Supervising those who support functions such as reporting, reporting writing, medical records, audits of client records, and the like. Will perform the “in the weeds” duties as they relate to key issues regarding agency reports, external audits, medical records, training of new and existing staff, policies and procedures, and continuous quality improvement.

Will be the agency liaison to community partners regarding programmatic and clinical audits, medical records, subpoenas and/or court orders and hospital liaison services. Will oversee the client satisfaction surveys and data reporting. Will manage the clinical help desk each day, Will develop agendas and lead meetings for agency CQI and closely related committee meetings.

Qualifications:

A master’s degree in Human Services, Psychology, Counseling, Social Work, Marriage and Family Therapy or closely related field.

Preferred candidate will hold an LPC, LCSW or LMFT in the State of Georgia

Must have prior supervisory experience
