As an Occupational Therapist, you will assess and treat residents to improve function, independence, and overall quality of life. This role includes developing evidence-based treatment plans, delivering skilled therapy services, and collaborating closely with the interdisciplinary team.
Key Responsibilities
- Evaluate residents to determine functional impairments and therapy needs.
- Develop and implement individualized treatment plans focused on ADLs and IADLs.
- Track progress and modify treatment plans as needed in coordination with the IDT.
- Recommend adaptive equipment and home modifications to promote independence.
- Educate patients, caregivers, and families on therapeutic strategies.
- Ensure accurate, timely documentation per state, federal, and facility guidelines.
- Participate in team meetings and care planning activities.
- Adhere to all regulatory and facility policies.
- Active Texas Occupational Therapist license.
- In good standing with all applicable licensing boards.
- SNF or long-term care experience preferred.
- Familiarity with Net Health is a plus.
- Strong communication and interpersonal skills.
