Responsibilities:

• Provide direct patient care to our patients, including administering medications, wound care, and other nursing procedures

• Develop and implement individualized care plans, collaborating with our interdisciplinary team to ensure patient-centered care

• Monitor and report patient status, identifying and addressing any changes or concerns

• Educate patients and their families on disease management, treatment options, and preventive care

• Maintain accurate and up-to-date patient records, adhering to HIPAA standards

• Participate in quality improvement initiatives to enhance patient outcomes and staff satisfaction

• Collaborate with our rehabilitation team to develop and implement individualized rehabilitation plans

Requirements:

• Graduation from an accredited Licensed Vocational Nursing or Registered Nursing program

• Current LVN or RN licensure in the state of Texas

• Ability to lift, push, and pull up to 50 pounds

• Strong communication and interpersonal skills

• Ability to work in a fast-paced environment, prioritizing multiple tasks and responsibilities
