Licensed Practical Nurse
We are looking for a reliable and detail-oriented Licensed Practical Nurse to monitor and provide support for patients enrolled in our Remote Patient Monitoring and Chronic Care Management program. This role will review daily patient data such as CPAP/BiPAP readings, blood pressure, pulse oximetry, glucose, and weight. This role will identify any abnormal readings and escalate to the patients provider according to established protocols. He/she will communicate regularly with patients through phone calls and messages, educate patients on proper CPAP maintenance, sleep hygiene, and lifestyle changes to help improve symptoms. This position is important with helping patients stay engaged, improving health outcomes, and supporting providers with consistent, real-time data.
Qualifications:
- Active LPN license
- 1 years in a clinical setting; experience in sleep medicine, pulmonology, or home health is highly preferred
- Strong understanding of chronic conditions such as obstructive sleep apnea, hypertension, diabetes, COPD, and heart disease
- Proficiency with EHR systems, Microsoft office and ability to learn remote patient monitoring software
- Exceptional phone etiquette and the ability to motivate patients toward long-term therapy adherence
Schedule: Monday – Friday | 8:30 AM – 5:00 PM (No weekends or holidays)
Pay: $22.00 - $26.00 per hour
