Role Description The PRN Nurse is a full-time, on-site role based in Milwaukee, WI. This role provides nursing oversight for personal care services, conducts client assessments, and develops and updates individualized plans of care in alignment with Medicaid and agency standards. Day-to-day responsibilities include monitoring client health status, documenting clinical findings, coordinating with personal care workers, and communicating with families, case managers, and other healthcare professionals. The PRN Nurse will provide education and support to staff, ensure care quality and safety, and participate in compliance and documentation audits as needed. The role may also involve occasional home visits to evaluate care delivery and support care coordination.
Qualifications
- Active Registered Nurse (RN) license in the state of Wisconsin and familiarity with Medicaid personal care regulations.
- Experience in home care, community health, geriatric care, or disability services, preferably within a diverse urban setting.
- Strong clinical assessment, care planning, and documentation skills, including proficiency with electronic health records.
- Effective communication and collaboration skills for working with clients, families, personal care workers, and interdisciplinary teams.
- Ability to prioritize tasks, manage time independently, and maintain professional boundaries and confidentiality.
- Comfort with providing guidance and training to direct care staff and supporting quality improvement initiatives.
