Position Summary:
Aviata Health Group is seeking a knowledgeable and detail-oriented MDS RN to join our clinical team. The MDS RN plays a critical role in ensuring accurate resident assessments, care planning, and regulatory compliance, while supporting quality outcomes and appropriate reimbursement.
Key Responsibilities:
- Complete and coordinate the Minimum Data Set (MDS) assessments in accordance with federal and state regulations
- Ensure accuracy and timeliness of all MDS submissions
- Coordinate interdisciplinary team (IDT) participation in the assessment and care planning process
- Develop, review, and update resident care plans based on MDS findings and clinical assessments
- Monitor case mix index (CMI) and support optimization of reimbursement
- Ensure compliance with Medicare, Medicaid, and other payer requirements
- Participate in utilization review and quality assurance performance improvement (QAPI) activities
- Collaborate with nursing staff, therapy, and physicians to ensure appropriate documentation
- Identify and address potential coding or documentation issues
- Maintain up-to-date knowledge of regulatory changes and MDS guidelines
- Assist with audits and surveys as needed
- Active Registered Nurse (RN) license in good standing
- Previous MDS experience in a skilled nursing or long-term care setting preferred
- Strong knowledge of RAI Manual, Medicare, and Medicaid guidelines
- Excellent assessment, analytical, and documentation skills
- Strong communication and team collaboration abilities
- Ability to manage multiple deadlines and priorities
- Experience in Skilled Nursing/Rehabilitation facilities preferred.
- One Year of experience as a MDS Coordinator.
- RAC-CT or RNAC preferred.
