Job Summary:

Responsibilities:
- Assist in implementing individualized treatment plans for each assigned resident
- Supervise Certified Nursing Assistants (CNAs) and participate in their evaluation
- Oversee Falls Program and Pressure Sore Program
- Provide nursing services
- Conduct and document thorough evaluations of each resident's medical status upon admission and throughout treatment
- Perform other duties as assigned
- Must possess a current, unencumbered, active state license to practice as an LPN
- Experience in Skilled Nursing/Rehabilitation facilities preferred
- Must be qualified, compassionate, and dedicated to a job well done
- $32 - $32 HOURLY salary
- 500 referral bonus for nurses and c.n.a's - paid out in 90 days
- Perfect Punch Program - $225 bi-weekly bonus (full time only)
- 401k with a match
- Shift diffs M-F after 3p and all shifts on the weekend
- Pay rate: Competitive, with additional holiday pay and overtime hours possible
