Job Summary
The ideal candidate will play a crucial role in providing nutritional care to patients in long-term care facilities. Duties
Conduct comprehensive patient assessments to evaluate nutritional needs and develop individualized care plans.
Provide guidance on tube feeding systems for patients requiring specialized nutrition support.
Collaborate with healthcare professionals in acute care settings to create and implement dietary interventions.
Supervise dietary staff and ensure compliance with hospital policies and procedures.
Offer health coaching to patients, focusing on weight loss strategies and lifestyle modifications.
Monitor patient progress and adjust nutrition plans as necessary based on ongoing assessments.
Educate patients and families about nutritional practices and healthy eating habits.
Maintain accurate documentation of patient interactions, assessments, and progress notes.
Requirements
Bachelor’s degree in Nutrition, Dietetics, or a related field from an accredited institution.
Registered Dietitian Nutritionist (RDN) credential is required.
Experience in acute care settings or long-term care facilities is preferred.
Strong knowledge of tube feeding systems and dietary management practices.
Excellent communication skills with the ability to educate patients effectively.
Proven ability to supervise staff and work collaboratively within a multidisciplinary team.
Familiarity with health coaching techniques and weight loss programs is a plus.
