About the Role
We are seeking a compassionate and skilled LPN to join our team at Avera Oahe Manor. In this role, you will provide professional nursing care to residents, including assessing their needs, developing care plans, and implementing interventions. You will also work closely with our interdisciplinary team to ensure that residents receive comprehensive care that meets their physical, emotional, and spiritual needs.
Responsibilities
The successful candidate will be responsible for:
• Performing all licensed practical nursing functions and services as directed by the Registered Nurse to all residents;
• Completing assessments on residents, making appropriate interventions, applying knowledge, prioritizing resident care, and using the nursing process (assess, plan, implement, evaluate);
• Complying with all pharmaceutical policies and procedures, ensuring that all medications/treatments are given safely and accurately in accordance with the physician's orders;
• Administering medications according to facility policy, competent in Pyxis medication administration system;
• Participating in the care planning process and assuring that resident care is implemented consistently and appropriate to the age of those served;
• Initiating and updating plan of care as appropriate;
• Complying with standards of professional documentation;
• Communicating and managing the delivery of resident care, ensuring quality care is provided;
• Guiding staff in meeting the needs of the residents, practicing routines based on residents' needs;
• Participating in physician rounds, communicating changes in resident condition to physician utilizing SBAR communication tool;
• Processing and transcribing physician's orders and implementing according to facility policy;
• Seeking educational opportunities, sharing knowledge with others, assisting with orientation of new staff;
• Investigating incidents, patient/family complaints, and making immediate report to VP of Resident Care or Administrator on-call;
• Facilitating conflict management while respecting diversity of people, their strengths, and ideas;
Requirements
To be considered for this role, you must have:
• An active license in the state of practice Upon Hire;
• Less than 1 year experience in Long Term Care preferred;
• The ability to work rotating days/evenings/nights/weekends;
• Strong communication and interpersonal skills;
• Ability to prioritize tasks and manage time effectively;
• Commitment to maintaining confidentiality and adhering to safety principles and regulations.
