SCHEDULE: (Full-Time) Monday-Friday (8AM - 4PM)
General Duties:
- will comply with all agency policies and procedures
- will advocate for the health & well being of all program consumers
- will report any unsafe or hazardous condition immediately
- will report any suspected neglect or abuse immediately
- will complete required training hours
- will work hours as stated on the Job Description
- will perform other duties as assigned by supervisor
- will maintain a safe and positive environment
Program Specific Duties:
- will accept caseload assignment from Lead Program Specialist
- will develop consumer centered Individualized Plan for each assigned individual.
- will schedule staffing meetings and make needed contacts concerning attendance at the staffing.
- will develop and update an on-going strength/needs/interest list to be used in the development of the IPP.
- will be responsible for the coordination and completion of assessments.
- will be responsible for the development of planned program activities.
- will be responsible for the development of goal plans, including target dates, short term objectives, means of assessment and criteria for completion of goals.
- will attend and facilitate interdisciplinary team meetings for assigned consumers.
- will work closely with assigned consumers and their Direct Care staff to insure that all plans are being executed.
- will be available during the hours of program operation to offer guidance and instruction in accurate implementation of goals & behavior intervention plans.
- will keep all records required by DHS regulations, including record of reviews, program goals, monthly and quarterly reports, written notification of reviews and record of movement.
- will work closely with parents and/or advocates to stimulate support and interest in the individual’s program
- will make referrals to the Supervisor concerning existing needs which are not being met through the ISP process.
- will maintain consumer files.
- will travel to offsite program areas as required.
- will maintain Certified Investigator status and conduct investigations as assigned.
- will participate in Human Rights Committee
Requirements:
- A Bachelor’s Degree in Human Services from an accredited college or university and 2 years of direct work experience with individuals with IDD.
- acceptable results on all pre employment testing and background checks
- the ability to comply with all physical demands of the job, with or without reasonable accommodation
- above average computer skills.
- above average written and verbal communication skills
- A valid PA Driver’s License, acceptable driving record and access to own vehicle
- ability to lift and carry up to 50 pounds
- ability to work independently
